The City Clerk Department must be notified of changes in DBA prior to the change occurring. Please complete the Change in DBA Form to notify our office of an upcoming change in DBA. Once this form is completed, it will be reviewed internally. If approved, the license will be updated and reissued.
Update your License Contact Information
Anytime a business needs to communicate an update in their contact information, the License Contact Updates Form must be completed. It is the responsibility of the business to ensure their contact and ownership information is up-to-date with the City Clerk Department at all times.
License Surrender
For businesses that have closed or no longer wish to offer the service the establishment has a license for, must surrender or relinquish their license. By completing the Affidavit of License Surrender, the authorized representative acknowledges the business will no longer offer that service as of the date the form is submitted.
