This application should be used for businesses wanting to operate a food truck. Before applying for a City of Bloomington Food Truck License, applicants should obtain a McLean County Health Department Permit which must be submitted with the application.

Application and all supporting documents must be submitted at least 15 days before the anticipated start of business.

Application Supplemental Documentation

Please attach the following documents to your application upon submission:

Certificate of Liability Insurance

Must Include VEHICLE LICENSE PLATE # in Description
City of Bloomington, 115 E. Washington St., Bloomington, IL 61701 must be listed as the Certificate Holder

McLean County Health Department Permit

NOTE: Inspections from the Economic & Community Development Department may be required. Call (309) 434-2226 to find out if one is required for you.
