Please attach the following documents to your application upon submission: 

• A list of driver’s information including: names, driver’s license numbers and addresses of drivers registered with the applicant as of the date of the application who will be providing TNC services in the City of Bloomington and the Town of Normal
• A sworn statement under oath by the TNC that: (1) the TNC has conducted the background checks as required by Section 1209 of this Chapter on any existing TNC drivers and will conduct such background checks on any future drivers prior to being allowed to operate for the TNC; (2) that background checks will be updated on all TNC drivers on an annual basis; (3) that all TNC drivers have passed the TNC’s background checks and are not disqualified under Section 1209(b) of this Chapter; (4) that the TNC has ensured the vehicles of all TNC drivers have met the qualifications of Section 1214 of this Chapter; and (5) that the TNC and all TNC drivers have met the insurance obligations set forth in Section 1210 of this Chapter.
• The applicant's insurance policies as required by Section 1210 of the Transportation Network Providers Act.
• A plan on the provision of background checks as required by Section 1206(h).
• Corporations and Limited Liability Companies Only: Certificate (or full-page print including date) of Good Standing from the Secretary of State’s office: provide a copy of the certificate or a screen shot of the Secretary of State’s website indicating an active status.
