Please attach the following documents to your application upon submission:
- Event site plan provided by the applicant which includes:
- The identification of the areas where alcohol will be stored and sold/served;
- Access points (entrances, exits, gates, etc.);
- The dimensions of the area proposed for the extension of premises;
- Location of additional adequate sanitation facilities, if applicable;
- Fencing or barricading for controlled entrances and exits to the proposed area;
- Locations of any tents or stages, if applicable; and
- Locations for adequate parking
- If ample parking is not available on the premises, written permission from neighboring businesses or residents must be sought via an event parking agreement. The event parking agreement must include written permission from the neighboring business or resident willing to accommodate the overflow parking needed for the event, as well as dates/times of the event and either the business name with permission granting the person’s name and title or the residential property owner’s name; it must be signed by the same person; and the signer’s contact information must also be included.

- Certificate of Insurance (Not Policy Declaration Page) The Certificate must show the establishment has liquor liability insurance and must include the following:
- Applicant’s name as the insured (e.g. if the applicant is a corporation, the corporation’s name must be listed);
- The date, times, and address of the location where the liquor is being consumed;
- The dates of coverage and the coverage limits
